Seeking an Internal Medicine (or Family Medicine) physician to provide value-based primary and post-acute care for senior living residents across skilled nursing, assisted/independent living, memory care and affordable housing sites. Responsibilities include admissions management, patient visits, rounding, documentation, and multidisciplinary team collaboration with flexible full- or part-time/PRN schedules, limited call, and opportunities for medical directorship. Requires MD/DO, board certification in Internal or Family Medicine, unrestricted state license, EMR proficiency; benefits include insurance, CME, paid malpractice, 401(k) match and supportive administrative resources.
